Cleaners' back syndrome

Udah pernah dengar nama penyakit yang satu ini? Cleaners' back syndrome- dari namanya saja sudah bisa ditebak kalau prevalensi tertinggi penyakit ini di temukan pada cleaners (walaupun tidak tertutup kemungkinan juga di temukan pada populasi lain he..he..). Belakangan ini insiden cleaners' back syndrome mengalami peningkatan yang significant (dilihat dari semakin banyaknya keluhan yang masuk). Kurang lebih 30% dari total populasi cleaners di UQISA mengalami syndrome ini yet hanya kurang dari 10% yang mencari penanganan medis. Selebihnya mencari pengobatan alternatif seperti di urut atau tidur aja seharian

Cleaners' back syndrome merupakan kumpulan gejala atau keluhan yang dirasakan oleh cleaners terutama setelah bekerja dan menggendong vacuum selama kurang lebih 4 jam (terhitung mulai dari hari minggu malam dan biasanya memuncak pada hari rabu malam).
Manifestasi gejala dan keluhan berupa:
  • Rasa tegang di sekitar pundak dan menjalar ke arah leher.
  • Nyeri di daerah punggung yang di aggravasi dengan perubahan posisi (terutama bila tunduk ke depan untuk mengambil vacuum)
  • Rasa mual terutama ketika melihat vacuum dan alat-alat kebersihan lainnya yang tidak ada hubungannya dengan kehamilan, mabuk perjalanan, hang-over dan penyakit organis lainnya sperti tht, radang selaput otak de.el.el
  • Tidak ada riwayat nyeri punggung sebelumnya (baca: sebelum menjadi cleaner).

Penanganan:

  • bila gejala tidak berat, syndrome ini bisa di atasi dengan istirahat yang cukup (intensitas cukup itu relative untuk tiap orang, ada yang bisa hanya dengan 2 jam, tapi ada yang harus sampai 8 jam. Idealnya 5-6 jam..gitu lho!)
  • bila gejala cukup berat, terapi istirahat harus di barengi dengan massage dan stretching pada daerah yang teraffected. Bagaimana tehnik massage yang bagus bisa di tanyakan pada ahli massagenya, dan bagaimana tehnik stretching yang bagus juga bisa di tanyakan pada ahlinya.
  • Bila gejala berlanjut, mungkin anda harus mempertimbangkan untuk meneruskan career di bidang ini...ingat pepatah: karena pulut binasa santan, tapi jangan karena vacuum binasa badan....

Kebelet pipis?

Pernah tidak mengalami kejadian seperti ini :
  • pulang dari mall rasanya biasa aja, tapi begitu sampai di depan pintu rumah..ehh tiba-tiba pengen pipis, trus kasak-kusuk nyari kunci pintu (akhirnya dapat) sambil goyang (karena udah pengeeen banget pipis).., masuk ke dalam rumah..tapi belum lagi sampai di toilet....ehh ternyata udah bocor duluan..(untung sudah di rumah ya???)
  • atau yang ini: ketemuan ama teman lama di mall.. trus ngerumpi sambil ngakak ha..ha..hi.hi, eh..tidak tahunya pas ngakak-nya ke-kencengan...kok kayak ada yang "basah" di bawah...

Anda mungkin menganggap itu adalah hal yang biasa dan tidak perlu terlalu dipusingin, hingga merasa tidak perlu mencari tahu. Tapi gimana nih..jika "kebelet pipisnya" ditengah-tengah meriahnya pesta, atau pas lagi berdiri manis-nis di atas podium dengan puluhan pasang mata menatap kagum ke arah anda...trus "bocor"...

Nah..untuk menghindari hal-hal yang seperti ini, sebaiknya kenali diri anda semenjak dini. Bisa jadi hal-hal seperti di atas terjadi karena anda sebenarnya terserang "inkontinentia urin" atau gak bisa nahan pipis trus ngompol....(bisa jadi kan??).

Salah satu faktor penyebab inkontinensia urin ini adalah lemahnya otot dasar panggul (pelvic floor muscle). Otot ini dalam keadaan normal seharusnya bisa mengatur kerja katup saluran kemih (sphincter), menyanggah organ-organ internal panggul (yang mungkin lumayan berat...apalagi kalau sementara hamil...wuiihh) dan menghadirkan sensasi "nyaman" pada saat berhubungan seksual. Nah kebayangkan kalau otot-otot ini menjadi lemah dan tidak bekerja sebagaimana mestinya?

Faktor-faktor yang bisa memicu kelemahan ini antara lain seperti udah pernah melahirkan berulang-ulang (multipara), punya riwayat infeksi saluran kencing, merokok, kegemukan, batuk kronik atau sembelit yang berulang.

Penelitian terbaru menyimpulkan bahwa kelemahan otot dasar panggul ini bisa di atasi dengan melatih secara rutin otot dasar panggul tersebut (pelvic floor muscle training/PMFT) plus core exercise untuk otot perut yang berhubungan dengan otot panggul.

Kegel's exercise for urine incontinentia

Tibaek et al (2005) found a positive effect of pelvic floor muscle training (modified Kegel' exercise) in 26 women with UI after stroke such as reduced frequencies of voiding and urine leakage, and improved pelvic floor muscle function, strength, static and dynamic endurance.

Another study by Smith et al (2007) revealed that UI is not only link to the weak of pelvic floor muslces, by using electromyographic they successfully profed that this problem is also related to weaken in abdominal muscles. Therefore, the exercise therapy should be targeted these two muscle groups.
1. Goals:

  • to improve the strength of pelvic floor muscles and abdominal muscles
  • to regain the urethral-closing reflex by mean increasing the number of motor units and motor learnig trough cerebral cortical facilitation

2. What do you need?

  • self-motivation
  • pillows
  • a mat
  • a quiet room (some people like to on relaxing music)

3. Preparation

  • lie down on your back in a quiet room where you're be able to relax and focus (far from unexpected disruption)
  • Place one pillow under your knees (this will allow you to feel relax and help you to contract the right muscles)
  • concetrate on your breathing. Do not holding your breath, let the air flows normally so that you will not deprive your muscle from taking oxygen.
  • Breath in when you relax and breath out when your muscle is contracting

4. Isolate your pelvic muscles

  • Relax your abdominals: to check if your abdominal muscles is relax, put your hand lightly on your tummy and make sure it is not tightening when you contract your pelvic floor
  • Relax your gluteals: Gluteals are the large muscle that make up your buttocks. The pelvic floor is locating between the right and the left buttock. To relax your gluteals, make sure you do not clenching your gluteals as well as do not litfs up your body off the ground when lying down on your back
  • Relax your hip adductors: Let your knees relax by resting them against each other.

5. Exercise:

  • We use the technique of 5 repetitions x 5 sets of long contractions and 5 repetitions x 5 sets of quick contractions in between.
  • Start doing the exercise very slowly and increase gradually
  • Try to complete 10 sets troughout the day
  • Increase the intensity if the exercise becomes easier

Long holding (LH) contractions:

  • squeeze your pelvic floor muscle tightly and hold it for 3-5 second
  • have a resting for 6-10 seconds
  • repeat five times
  • this equals 1 set

Quick holding (QH) cotractions:

  • squeeze your pelvic floor muscle for 1 second
  • have a resting for 1 second
  • repeat five times
  • this equals 1 set

a sample of complete program:

  • Morning: LH-QH-LH-QH-LH-QH + Abdominal exercise
  • Before dinner: LH-QH-LH-QH-LH-QH
  • Before night sleeping: LH-QH-LH-QH-LH-QH

Urine Incontinentia and exercise therapy

Urine incontinentia is defined as involuntary leakage of urine during light stressess that increase intraabdominal pressure such as sneezing, coughing or even laughing (Abrams et al., 2003).This is thought to be related to deficits in the active and passive structure around pelvic floor and become a significant problem among women as this can affect their working performance, social and sexual life. Statistically, over 50% of women populatin across over the world suffer from urine incontinentia yet less then 20% seeking for health intervention. Why so? becuse most women thought that this is a personal-embarassing problem. Women with urine incontinentia have been proofed to get improvement from exercise therapy (known as Kegel's exercise) involving their pelvic floor musculature and other accessories muscle of the pelvic as well as urgency delay and relaxation techniques.
